Identity and contact data: name, surname, telephone number, email address; where a patient record is created, national identification number, date of birth and address.
Health data (special category): medical history, current medication, allergies, chronic conditions, intraoral examination findings, radiographs, treatment plan and records of procedures performed.
Security data: IP address and submission time of forms sent through this website, retained for a limited period to prevent misuse.
Images: intraoral and facial photographs, taken only where necessary for treatment planning and with your explicit consent.
The appointment and contact forms on this website do not ask for health data. Please do not describe your medical situation in the form; share it verbally at the examination.
Identity and contact data are processed on the grounds of Article 5/2-(c) (performance of a contract), Article 5/2-(ç) (legal obligation) and Article 5/2-(f) (legitimate interest) of the KVKK.
Health data are processed under Article 6/3 of the KVKK for the purposes of protection of public health, preventive medicine, medical diagnosis, treatment and care services, by healthcare personnel bound by a duty of confidentiality. Any processing beyond that scope requires your explicit consent.

Requests submitted through the forms on this website may be processed via the infrastructure of a form delivery provider acting on the clinic's behalf, so that the message reaches the clinic and can be answered.
Your treatment and health data are never sold or shared with third parties for marketing purposes.
Data are collected partly by automated and partly by non-automated means: through the appointment and contact forms on this website, telephone and WhatsApp conversations, patient information and consent forms completed at the clinic, and the medical records generated during examination and treatment.
Patient files and medical records are retained for the minimum periods prescribed by health legislation (as a rule, 20 years from the last procedure). Financial records are kept for five years under the Tax Procedure Law, and technical data relating to form submissions for no longer than the purpose requires. At the end of the period data are deleted, destroyed or anonymised.
Under Article 11 of the KVKK you have the right to learn whether your personal data are processed; to request information if they are; to learn the purpose of processing and whether the data are used accordingly; to know the third parties to whom data are transferred in Türkiye or abroad; to request correction of incomplete or inaccurate data; to request erasure or destruction; to request that such action be notified to the recipients of the data; to object to a result arising against you from analysis solely by automated means; and to claim compensation for damage arising from unlawful processing.
You may submit requests in writing, in accordance with the Communiqué on the Procedures and Principles of Application to the Data Controller, to Noter Sok. No:18/3, Caddebostan, Kadıköy / İstanbul or to info@dttunaklinik.com. Applications are concluded within 30 days at the latest.
