What it is
A fully edentulous jaw does not need an implant for every tooth. Four implants (six in some cases), placed anteriorly and at an angle, can support a fixed prosthesis spanning the whole arch. Angled placement can reduce the need for grafting despite bone loss at the back of the jaw.
Who is it for?
Patients who have lost all their teeth, are unhappy with a removable denture, or whose remaining teeth all need removing. Suitability is determined on tomography, taking into account bone volume and density, gum health and general medical status.
Uncontrolled diabetes, heavy smoking and certain medications (bisphosphonates in particular) increase risk and are discussed separately.
The process
- Planning. Three-dimensional tomography, intraoral scan and prosthetic planning together. Implant positions are decided by where the prosthesis needs to sit — not the other way round.
- Surgery day. Any necessary extractions, implant placement and, in most cases, a fixed temporary prosthesis the same day.
- Healing. Three to six months, with soft food and meticulous cleaning.
- Definitive prosthesis. Impressions are taken after healing and the final prosthesis made.
Expectations and care
A fixed prosthesis makes a clear difference to chewing compared with a removable denture. In return it demands more careful maintenance: special brushes and a water flosser are needed to clean beneath it. Removal for professional cleaning at least once a year is advised.
No treatment can be guaranteed for life; success is directly linked to bone, hygiene and smoking.

After treatment
- Keep to soft, lukewarm food for the first week.
- Apply cold for the first 24 hours; avoid heat.
- Smoking directly impairs healing — stop for at least two weeks.
- Use the brushes and water flosser advised to clean beneath the prosthesis.
- Take all prescribed medication as directed.
- Attend review appointments; they are more frequent in the first year.
