Why it is needed

After a tooth is removed the bone in that area resorbs over time; a site left empty for long may no longer have the height or width an implant needs. In the upper posterior region the maxillary sinus also expands downwards, thinning the bone further.

In these situations the bone must be augmented before an implant can be placed.

Methods

Bone grafting. Graft material is placed in the deficient site and covered with a membrane. The material acts as a scaffold for your own bone to form, and is gradually replaced by it.

Sinus lift. In the upper posterior region the sinus membrane is carefully elevated and graft placed in the space beneath. Depending on the remaining bone height, the implant is placed at the same time or four to six months later.

Process and healing

The procedure is performed under local anaesthesia. Swelling and mild pain over the first two to three days are expected. After a sinus lift you should additionally avoid blowing your nose forcefully, keep your mouth open when sneezing, and avoid flying and diving for a period — pressure changes can affect healing.

Healing usually takes four to six months, after which the implant is planned.

What to know

Grafting means an additional stage and additional time. Knowing that from the outset removes the "I came for an implant, why am I still waiting?" question. Total duration and stages are set out in writing at the planning appointment.

After treatment

Apply cold for the first 24 hours and avoid heat.
After a sinus lift, do not blow your nose forcefully and keep your mouth open when sneezing.
Avoid flying and diving for two weeks, or as advised.
Smoking directly lowers graft success — stop for at least two weeks.
Take prescribed antibiotics and medication in full.
Call the clinic if you have a nosebleed, increasing swelling or fever.
