Space Maintainers

Why it is needed

Milk teeth hold space for the permanent teeth developing beneath them. If a primary molar is lost early through decay or trauma, the teeth behind drift into the gap. By the time the permanent tooth erupts the space has narrowed, leading to crowding, impaction or a need for orthodontic treatment later.

A space maintainer breaks that chain with a simple, low-cost appliance.

When it is not needed

If the permanent tooth is close to erupting — advanced root development and thin overlying bone on the radiograph — a maintainer may not be necessary. The decision is made on clinical examination together with radiographs; not every early tooth loss requires one.

Types

Depending on which tooth was lost and how many, fixed appliances (band and loop, lingual arch) or removable ones are used. Fixed maintainers do not depend on the child's cooperation and cannot be taken out; removable ones are easier to clean but require discipline.

Follow-up

The maintainer stays in place until the permanent tooth begins to erupt, and is checked at six-monthly visits. If it breaks or comes out, book an appointment without waiting: the space can close within a few months.
